## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON TAKEOFF FROM RUNWAY 24 JUST AFTER ROTATION THE AIRCRFAFT IMPACTED THE RUNWAY. ALL THREE LANDING GEAR COLLAPSED AND THE AIRCRAFT SLID THROUGH THE GRASS ON THE RIGHT SIDE OF RUNWAY 24. THE RIGHT WING STRUCK THE TAXI SIGN FOR A-1 AND THE LEFT WING STRUCK TAXI AND RUNWAY LIGHTS. THE LIGHTED SIGN AND ONE TAXI AND ONE RUNWAY LIGHT WERE DAMAGED. THE AIRCRAFT CAME TO REST AT THE INTERSECTION OF RUNWAY 24 AND TAXI A-1. THE PILOT WAS ABLE TO EXIT THE AIRCRAFT AND DID NOT REPORT ANY INJURY.

THE AIRCRAFT RIGHT WING WAS SEVERELY DAMAGED FORM IMPACTED WITH THE LIGHTED TAXI SIGN. THE AIRCRAFT FIREWALL WAS SEVERELY DAMAGED FROM THE IMPACT. THERE WAS ALSO MAJOR DAMAGE TO THE RIGHT WING.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB GAA17CA567](https://openairdocs.org/ntsb/GAA17CA567), Sep 6, 2017: Chancey Gerry M RV-12 (N1214J), Dunkirk, NY, substantial damage. Probable cause: The pilot's failure to maintain lateral/bank control during takeoff in crosswind conditions.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
