## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE AIRCRAFT WAS CONDUCTING A MEDICAL FLIGHT WITH TWO CREW, TWO MEDICAL PERSONNEL, ONE PATIENT, AND ONE COMPANION. DURING THE FINAL WALK-AROUND WITH AIRCRAFT LOADED THE SIC DISCOVERED A LOW TIRE (#1 LH O/B). THE CREW MADE CONTACT WITH THE FBO WHO CONTACTED A MECHANIC ON THE FIELD TO SERVICE THE AIRCRAFT. THE MECHANIC SERVICED BOTH LH MAIN TIRES WITH NITROGEN WHILE LOADED AT THE DIRECTION OF THE CREW, WHO ADVISED TO AIR THEM UP TO 160 PSI. ONCE SERVICED, THEY PROCEEDED ON THEIR PLANNED TRIP. DURING THE TAKEOFF ROLL JUST BEFORE V1 THEY HAD A BLOW-OUT ON THE LH SIDE AND ABORTED THE TAKEOFF. THEY APPLIED MAX BRAKING WHICH CAUSED A MINOR BRAKE FIRE ON THE RH SIDE DAMAGING THE GEAR STRUT. REMNANTS OF THE TIRE, BRAKES, AND/OR WHEEL ON THE LH SIDE STRUCK THE UNDERSIDE OF THE WING RESULTING IN DAMAGE TO THE SKIN OF THE AIRCRAFT. THE AIRCRAFT CONTINUED OFF THE END OF THE RUNWAY INTO A GRASSY AREA ON THE DEPARTURE END AND CAME TO A STOP, NO DAMAGE TO THE SURFACE OCCURED. THE CREW EVACUATED THE PASSENGERS FROM THE AIRCRAFT AND EXTINGUISHED THE FIRE WITH AN ON-BOARD FIRE EXTINGUISHER.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
