## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

REPUBLIC AIRLINE FLIGHT 5894 DEPARTED NASHVILLE INTERNATIONAL (BNA) AIRPORT ENROUTE TO LAGURDIA (LGA) AIRPORT. THE AIRCRAFT HIT A LARGE BIRD AFTER ROTATION, AND CONTINUED TO LGA, WITHOUT INCIDENT. UPON ARRIVAL, MAINTENANCE PERFORMED A BIRD STRIKE INSPECTION. DURING THE INSPECTION, MAINTENANCE FOUND A LARGE DENT IN THE LEADING EDGE SLAT BETWEEN THE LEFT HAND ENGINE AND FUSELAGE. THE DENT EXCEEDED THE STRUCTURAL REPAIR LIMITS. FOR REPAIRS, THE AIRCRAFT FLEW TO PORT COLUMBUS INTERNATIONAL (CMH) OHIO, ON A SPECIAL FLIGHT PERMIT. AIRLINE MAINTENANCE PERSONNEL RETURNED THE AIRCRAFT TO SERVICE AFTER REPLACING THE DAMAGED SLAT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
