## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE DUPAGE FSDO WAS NOTIFIED OF PARACHUTE INCIDENT: ON JUNE 15, 2018, REGINALD HURLBUT SUSTAINED FATAL INJURIES FROM A SKYDIVING INCIDENT AT SKYDIVE CHICAGO, OTTAWA, ILLINOIS, FOLLOWING A WINGSUIT/FORMATION JUMP. WITNESSES OBSERVED AND GO-PRO VIDEO FOOTAGE CONFIRMED THAT MR. HURLBUT'S PARACHUTE DEPLOYED NORMALLY BUT IMMEDIATELY BEGAN SPINNING TO THE LEFT, CAUSING TWISTING OF THE CANOPY LINES. THIS RENDERED THE MAIN CHUTE INEFFECTIVE AND THE PARACHUTIST FELL TO THE GROUND. THE MAIN CHUTE WAS NOT JETTISONED AND THE EMERGENCY CHUTE WAS NOT DEPLOYED PRIOR TO IMPACT. EMERGENCY RESPONDERS ATTEMPTED RESUSCITATION EFFORTS WITHOUT SUCCESS.

DOCUMENTATION RECOVERED AT THE SCENE IDENTIFIED MR. HURLBUT AS A CANADIAN CITIZEN AND CANADIAN SPORT PARACHUTING ASSOCIATION MEMBER #5232.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
