## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

BASED ON THE PILOT?S STATEMENT, THE AUTOGYRO (GYROPLANE) WAS ATTEMPTING TO TAKEOFF FROM RUNWAY 17 BUT WAS UNABLE TO ENGAGE THE ROTOR DISC PRE-ROTATOR. THE CONTROL TOWER REQUESTED THAT HE EITHER BEGIN HIS TAKEOFF OR TAXI CLEAR OF THE RUNWAY. THE PILOT TAXIED OFF THE RUNWAY, RETURNED TO THE RAMP AND TESTED THE ROTOR DISC PRE-ROTATOR AND WAS ABLE TO ENGAGE IT; THE PILOT THEN REQUESTED TAXI AND TAKEOFF CLEARANCE. AFTER RECEIVING CLEARANCE HE TAXIED ONTO THE RUNWAY AND ENGAGED THE PRE-ROTATOR AND PRE-ROTATED THE ROTOR DISC TO 200 RPM. HE THEN BEGAN HIS TAKEOFF ROLL, AT ABOUT 10 FEET ABOVE THE RUNWAY HE FELT A STRONG VIBRATION, THE AIRCRAFT LOST ALTITUDE, IMPACTED THE RUNWAY, ROLLED OVER ON ITS LEFT SIDE AND SLID ALONG THE RUNWAY. BOTH HE AND HIS PASSENGER WERE ABLE TO EXIT THE AIRCRAFT WITH HELP FROM A GOOD SAMARITAN AND BOTH OCCUPANTS RECEIVED MINOR INJURIES.

AN INDIVIDUAL WHO WITNESSED THE TAKEOFF STATED THAT THE AUTOGYRO?S ROTOR DISC WAS FLAT DURING THE TAKEOFF ROLL. THE AUTOGYRO?S FLIGHT MANUAL REQUIRES THAT THE CONTROL STICK BE FULLY AFT DURING TAKEOFF. A FULLY AFT STICK TILTS THE ROTOR DISC AFT. BASED ON THE PILOT?S STATEMENT AND OUR EXAMINATION OF THE AUTOGYRO IT APPEARS THAT THERE WERE NO MECHANICAL ISSUES WITH THE AUTOGYRO. BASED ON OUR INVESTIGATION IT APPEARS THAT THE PILOT FAILED TO PULL THE CONTROL STICK BACK TO ITS FULL AFT POSITION WHICH THEREFORE CAUSED THE ROTOR DISC TO FAIL TO REACH THE POSITION REQUIRED FOR A SAFE TAKEOFF.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR18LA190](https://openairdocs.org/ntsb/WPR18LA190), Jul 7, 2018: Autogyro Calidus (N623AG), Tacoma, WA, substantial damage. Probable cause: The pilot's failure to properly configure the gyrocopter for takeoff, which resulted in it developing high speed and led to a subsequent loss of control.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
