## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

*AT APPROXIMATLEY 1730 ON THE 1ST OF SEPTEMBER, THE FAA RECEIVED A CALL FROM TIPTON COUNTY SHERIFF, INFORMING THAT A SMALL AIRCRAFT CRASHED WITH ONE PERSON ON BOARD EAST OF THE COVINGTON AIRPORT, COVINGTON, TN.

*THE FAA ARRIVED ON SCENE, AT 1810. THE AIRCRAFT WAS AN EXPERIMENTAL AMATEUR BUILT, A JETEZ N815EY. THE AIRCRAFT'S PIC WAS LANCE HOOLEY.

*THE FOLLOWING DAY THE NTSB ARRIVED AT THE SCENE.

*INTERVIEWED SEVERAL WITNESSES. IN THE WITNESS STATEMENTS, THEY STATED THAT THE AIRCRAFT TOOK OFF FROM THE NORTH, TURNED RIGHT TO COME ACROSS THE RUNWAY.

*THE AIRCRAFT BEGAN TO CLIMB, THE WITNESS NOTICED THE LEFT WING STARTED TO FLUTTER, AND THE WING BEGAN TO COME APART. THE AIRCRAFT PITCHED UP AND THEN ROLLED 90 DEGREES TO THE LEFT. THE RIGHT WING THEN BEGAN TO COME APART AS WELL. THE AIRCRAFT THEN PITCHED DOWN, AND LANDED UPSIDE DOWN, AND A POST CRASH FIRE CONSUMED THE AIRCRAFT.

*THE AIRCRAFT WAS CERTIFICATED IN JUNE 2014. THE ORIGINAL BUILDER WAS LANCE HOOLEY, THE OWNER/OPERATOR ALSO DESIGNED THAT AIRCRAFT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA18FA240](https://openairdocs.org/ntsb/ERA18FA240), Sep 1, 2018: Lance M Hooley Jetez (N815EY), Covington, TN, destroyed, 1 fatal. Probable cause: The improper manufacture of the left wing, which resulted in a left wing flutter event and failure at low altitude and high speed.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
