## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

^PRIVACY DATA OMITTED^ WAS GIVING FLIGHT INSTRUCTION AT THE RICHLAND AIRPORT ON RUNWAY 14 PERFORMING TOUCH AND GO LANDINGS. DURING ONE OF THE TOUCH AND GO MANEUVERS, THE AIRCRAFT?S FLAPS REMAINED AT 40 DEGREES. THE AIRCRAFT TRAVELED TO THE RIGHT OF THE RUNWAY, THEN BACK LEFT ACROSS THE RUNWAY. AT 1728 FEET FROM THE DISPLACED THRESHOLD AND 7 FEET FROM THE RIGHT EDGE OF THE RUNWAY, THE LEFT WING TIP CONTACTED THE RUNWAY FOR 36 FEET. THE NOSE WHEEL CONTACTED THE RUNWAY 6 FEET RIGHT OF CENTERLINE BREAKING THE NOSE FORK AND WHEEL ASSEMBLY OFF. THE RIGHT WING TIP CONTACTED THE RUNWAY 9 FEET FROM THE LEFT EDGE FOR 4 FEET. THE AIRCRAFT DEPARTED THE RUNWAY WHERE THE NOSE FORK HIT A CONCRETE PYLON WHERE THE RUNWAY EDGE LIGHT STOOD. THIS CAUSED THE AIRCRAFT TO FLIP INVERTED COMING TO REST 26 FEET OFF THE LEFT SIDE OF THE RUNWAY.

THE AIRCRAFT HAD DAMAGE TO BOTH WINGS, VERTICAL STABILIZER, AND SUBSTANTIAL DAMAGE FROM THE FIREWALL FORWARD. THE GROUND ALONG THE PATH OF TRAVEL HAD GOUGE MARKINGS FROM THE NOSE FORK, PROPELLER, SPINNER AND COWLING.

THE AIRCRAFT WAS POSITIONED UPRIGHT AND TIED DOWN. WE CHECKED THE FLIGHT CONTROLS, ALL OPERATED NORMAL. THE ENGINE CONTROLS WERE CHECKED AND ONLY THE THROTTLE WOULD MOVE DUE TO THE DAMAGE TO THE ENGINE COMPARTMENT. THE BREAKS WERE TESTED FROM BOTH SETS OF RUDDER PEDALS ALL WERE NORMAL. FUEL WAS CHECKED AND NO CONTAMINANTS WERE FOUND, OIL LEVEL WAS AT 7QTS.

^PRIVACY DATA OMITTED^ WAS TAKEN TO THE HOSPITAL WITH HEAD LACERATIONS AS WELL AS HAVING A HEART ATTACK. IT IS NOT KNOWN WHEN THE HEART ATTACK PRESENTED ITSELF.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR18LA255](https://openairdocs.org/ntsb/WPR18LA255), Sep 4, 2018: Cessna 150 (N8028F), Richland, MO, substantial damage. Probable cause: A loss of directional control during a touch-and-go landing in gusting crosswind conditions.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
