## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

DURING TAKEOFF FROM RUNWAY 36, PILOT NUMBER TWO STATED HE BEGAN TO ROTATE AT APPROXIMATELY 65 MPH AND THE AIRCRAFT LIFTED OFF THEN SETTLED BACK DOWN. THE TAKEOFF WAS ABORTED AS THE AIRCRAFT LIFTED OFF AGAIN AND WOULD NOT CLIMB. AS THE AIRCRAFT SETTLED BACK DOWN, THE NOSE WHEEL HIT A DIP IN THE RUNWAY SURFACE CAUSING THE AIRCRAFT TO BOUNCE OFF CENTER LINE. THE AIRCRAFT THEN DEPARTED THE RUNWAY TO THE RIGHT AT A 020 HEADING. THE AIRCRAFT CAME TO A REST WITH THE NOSE GEAR COLLAPSED ABOUT 100 FEET EAST OF THE RUNWAY IN HIGH GRASS AND MUD. BOTH OCCUPANTS THEN EXITED THE AIRCRAFT AND WALKED AWAY WITHOUT INJURY. SUBSTANTIAL DAMAGE NOTED TO ENGINE LOWER FIREWALL AND LOWER BELLY SKIN AND UNDERLYING STRUCTURE. DAMAGE ALSO NOTED TO PROPELLER, BROKEN LOWER ENGINE MOUNTS, NOSE GEAR, AND LEFT WING TIP.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB GAA19CA438](https://openairdocs.org/ntsb/GAA19CA438), Jul 9, 2019: Beech 19 (N9161S), Columbia Station, OH, substantial damage. Probable cause: The pilot's failure to see and avoid tall grass along the side of the runway during the takeoff roll, which resulted in his loss of directional control, a runway excursion, and nose-over.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
