## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 10/02/2019 AT APPROXIMATELY 1345 A CFI AND STUDENT PILOT ROLLED HELICOPTER N4050X OVER DURING PRACTICE LANDINGS TO A TOWABLE LANDING PAD. ONE SUCCESSFUL LANDING, WITH THE STUDENT PILOT MANAGING THE COLLECTIVE/PEDALS AND THE CFI MANAGING THE CYCLIC, IMMEDIATELY PRECEDED THE MISHAP LANDING. N4050X DESCENDED WHILE BACKING AWAY IN A HOVER TO SET UP FOR LANDING CAUSING THE RIGHT SKID TO MAKE CONTACT WITH THE PAD. SUBSEQUENT CONTROL INPUTS RESULTED IN THE RIGHT SKID BECOMING A PIVOT POINT IN THE AFT LEFT QUADRANT OF THE TOWABLE PAD. A RIGHT ROLL RAPIDLY DEVELOPED, A CRITICAL ANGLE WAS EXCEEDED AND A DYNAMIC ROLLOVER OF THE HELICOPTER OCCURRED. THE HELICOPTER CAME TO REST WITH THE RIGHT SKID ON THE GROUND AND THE RIGHT SIDE OF THE FUSELAGE RESTING NEAR THE AFT LEFT CORNER OF THE LANDING PAD.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB GAA20CA002](https://openairdocs.org/ntsb/GAA20CA002), Oct 2, 2019: Robinson R44 (N4050X), Carlsbad, CA, substantial damage. Probable cause: The student pilot's incorrect collective and cyclic applications during landing and the flight instructor's delayed remedial action, which resulted in a roll-over.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
