## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

FROM NEIGHBORS SECURITY CAMERA VIDEO IT APPEARS THAT AS THE AIRCRAFT ROTATED IT WAS AT THE VERY LEFT EDGE OF THE GRASS RUNWAY. IT IMMEDIATELY ROLLED RIGHT THEN BEGINS TO ROLL TO THE LEFT. THE AIRCRAFT NEVER GAINS MORE THAN ABOUT 20 FEET OF ALTITUDE AS IT BEGINS TO TURN TO THE LEFT. THE AIRCRAFT IS IN A SKIDDING LEFT TURN WHEN IT APPEARS TO STALL AND ROLL TO THE LEFT AS IT IMPACTS TREES, BREAKS APART AND CATCHES FIRE. THE FUSELAGE CAME TO REST ABOUT 150 FEET FROM ITS FIRST IMPACT WITH THE TREES. THE PILOT WAS PULLED FROM THE AIRPLANE BY A NEIGHBOR WHO HEARD THE CRASH. THE AIRPLANE CAUGHT FIRE AND WAS MOSTLY CONSUMED BY A POST-CRASH FIRE.

ACCORDING TO THE PILOT, HE WAS ON THE TAKEOFF ROLL AND HAD GOT OFF CENTERLINE TO THE LEFT. HE APPLIED RIGHT RUDDER TO CORRECT AND HIS FOOT BECAME STUCK UNDER THE RUDDER PEDAL. HE BEGAN TO LOSE DIRECTIONAL CONTROL SO HE REDUCED THE POWER ON THE RIGHT ENGINE IN ORDER TO GET SOME DIRECTION CONTROL BACK. HE WAS STILL TURNING TO THE LEFT AND HE ADDED FULL POWER AGAIN TO TRY TO CLIMB OVER THE TREES BUT IT WAS TOO LATE AND HE HIT THE TREES AND CRASHED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR20LA111](https://openairdocs.org/ntsb/WPR20LA111), Mar 19, 2020: Piper PA 30 (N7155Y), Eagle Creek, OR, destroyed. Probable cause: The pilot's loss of airplane control during takeoff due to his left shoe’s interference with the rudder pedal.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
