## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 08/30/20 KCFD SECURITY CAMERAS OBSERVED THE SUBJECT AIRPLANE LOADING PILOT AND 3 PASSENGERS AT 1332 CDT. THE AIRPLANE HAD NOT BEEN REFUELED AT KCFD AND A COCKPIT PHOTO, APPARENTLY TAKEN DURING RUN-UP (AND POSTED ON SOCIAL MEDIA) REVEALED THE FUEL LOAD TO BE AN ESTIMATED 34 US GALLONS, APPROXIMATELY 203 LB. THE CABIN LOAD IS ESTIMATED TO BE 820 LBS INCLUDING BAGGAGE FOR AN ESTIMATED TAKE-OFF GROSS WEIGHT OF 2753 LB, 96% OF MAXIMUM GROSS WEIGHT. THE DENSITY ALTITUDE WAS APPROXIMATELY 3000 FT.

SECURITY CAMERAS RECORDED THE TAKE-OFF ROLL BEGAN AT 1346 CDT ON RUNWAY 15, BUT THE TAKE-OFF AND SUBSEQUENT ACCIDENT WERE BLOCKED FROM CAMERA VIEW. THE AIRPLANE LIKELY CRASHED DURING CLIMB OUT AND IMPACTED THE GROUND APPROXIMATELY 50 YARDS BEYOND THE DEPARTURE END OF RUNWAY 15 AND APPROXIMATELY 50 YARDS EAST (LEFT) OF THE RUNWAY'S EXTENDED CENTERLINE. THERE WAS NO POST-CRASH FIRE.

THE AIRPLANE APPEARS TO HAVE IMPACTED THE GROUND AT A HIGH ANGLE. THE ENGINE SECTION WAS TWISTED FROM THE AIRPLANE, AND WAS FACING ROUGHLY TO THE NORTH. INSPECTION OF THE PROPELLER INDICATED IT IS LIKELY IT WAS NOT UNDER POWER AT THE INSTANT OF IMPACT. ADDITIONALLY, THE RIGHT WING HAD CRUSH DAMAGE TO THE LEADING EDGE AND THE LEFT WING LEADING EDGE HAD NO SUCH DAMAGE. THE TAIL CONE AREA AND EMPENNAGE SEPARATED FROM THE FUSELAGE AFT OF THE CABIN AREA AND CAME TO REST IN THE WRECKAGE POINTING ALSO ROUGHLY NORTH. THE CABIN SECTION AND WINGS WERE INVERTED, OVERLAPPING THE ENGINE NACELLE AND TAIL CONE, FACING TO THE SOUTH. THE MAIN LANDING GEAR WERE STILL EXTENDED.

EXAMINATION OF THE CABIN AREA SHOWED THE FUEL SELECTOR TO BE IN THE DETENT FOR THE RIGHT MAIN FUEL TANK. EXAMINATION OF THE RIGHT WING SHOWED THE FUEL BLADDER TO BE EXPOSED BUT THE BLADDER WAS IN GOOD CONDITION WITH NO TEARS OR PUNCTURES. EXAMINATION OF THE RIGHT MAIN FUEL BLADDER FOUND NO FUEL IN THE BLADDER. FIRST RESPONDER REPORTS INDICATE THE NOTICEABLE LACK OF ANY APPARENT FUEL LEAKAGE AT THE ACCIDENT SITE.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN20LA370](https://openairdocs.org/ntsb/CEN20LA370), Aug 30, 2020: Piper PA24 (N7469P), Bryan, TX, substantial damage, 3 fatal. Probable cause: A total loss of engine power on takeoff due to fuel starvation.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
