## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AT 1525 EDT ON 04/06/2021 N61EF A BUSHBY MUSTANG 2 CRASHED AT KORH (WORCESTER, MA) AFTER TAKEOFF FROM RUNWAY 33. IN A WRITTEN STATEMENT THE PILOT IN COMMAND (PIC) STATED THAT THE AIRCRAFT LIFTED OFF NORMALLY AND WAS ACCELERATING, HE ESTIMATES THAT AT APPROXIMATELY MID FIELD AT AN ALTITUDE OF 6 FEET THE AIRCRAFT BEGAN TO ROLL TO THE RIGHT AND TURN TO THE RIGHT. HE APPLIED LEFT PRESSURE TO THE STICK BUT THERE WAS NO CONTROL RESPONSE. THE AIRCRAFT WAS NOW IN A RIGHT BANK OF APPROXIMATELY 30 DEGREES. AT THIS POINT THE PIC ABORTED THE TAKEOFF. THE AIRCRAFT IMPACTED THE GROUND ON THE GRASS BETWEEN RUNWAY 32 AND TAXIWAY BRAVO COLLAPSING BOTH MAIN LANDING GEAR. THE AIRCRAFT CAME TO A STOP ON TAXIWAY BRAVO. IN A WRITTEN STATEMENT THE INSTRUCTOR PILOT WHO WAS PROVIDING TRANSITION TRAINING STATED THAT AT 4 TO 6 FEET ALTITUDE THE PIC APPLIED LEFT AILERON TO STOP THE AIRCRAFT FROM ROLLING RIGHT. THE PIC PUT BOTH HANDS ON THE STICK PUSHING LEFT AND EXCLAIMED

"IT'S ROLLING RIGHT AND I CAN'T STOP IT." THE INSTRUCTOR PILOT THEN ATTEMPTED TO MOVE HIS STICK FULL LEFT AILERON, THE CONTROL STICK WOULD NOT MOVE LEFT. THE AIRPLANE CONTINUED ROLLING RIGHT TO A 30 DEGREE RIGHT BANK. THE INSTRUCTOR PILOT THEN PULLED THE THROTTLE TO IDLE AS THE AIRCRAFT STRUCK THE GROUND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA21LA177](https://openairdocs.org/ntsb/ERA21LA177), Apr 6, 2021: Forisch Edward Bushby Mustang M II (N61EF), Worcester, MA, substantial damage. Probable cause: Binding of the left aileron bellcrank push/pull rod, which resulted in an uncommanded right turn.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
