## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON 04/20/2021 AT 18:50 LOCAL TIME, AIRCRAFT N284ND WAS INVOLVED WITH AN OCCURRENCE AT MARICOPA ARIZONA (A39). IN AN INTERVIEW WITH THE PILOT HE MADE THE STATEMENT THAT THE AIRCRAFT VEERED SLIGHTLY TO THE LEFT DURING TAKE-OFF AND ATTEMPTS TO CORRECT BACK TO CENTER WERE NOT WORKING. THE PILOT MADE A DECISION TO GET AIRBORNE AND THEN MAKE NECESSARY CORRECTIONS. BEFORE BECOMING AIRBORNE THE AIRCRAFT SUDDENLY BEGAN TO SPIN TO THE RIGHT AS THE RIGHT WING TIP CONTACTED THE GROUND, THEN THE LEFT WING CONTACTED THE GROUND. AFTER SPINNING THE AIRCRAFT CAME TO A STOP, UPRIGHT BUT SEVERELY DAMAGED. THE PILOT AND PASSENGER WERE ABLE TO EGRESS WITHOUT INJURY. THE PILOT STATED THE AIRCRAFT WAS PERFORMING NORMALLY AND IT WAS A NORMAL TAKE-OFF ROLL.

IN AN INTERVIEW WITH THE AIRPORT MANAGER, WHO WITNESSED MOST OF THE EVENT, HIS STATEMENT WAS SIMILAR TO THE PILOT?S IN THAT HE HEARD METAL SCRAPING THE GROUND AND TURNED TO SEE THE AIRCRAFT SPINNING AROUND AS THE R/H MAIN LANDING GEAR HAD COLAPSED. THE AIRPORT MANAGER REPORTED THAT TWO RUNWAY LIGHTS WERE BROKEN IN THE EVENT, ONE ON THE LEFT SIDE OF THE RUNWAY AND ONE FURTHER DOWN THE RUNWAY ON THE RIGHT.

WHEN ASKED, THE PILOT STATED THAT HE DID NOT RECALL HITTING THE LIGHTS BUT BELIEVED IT WAS A POSSIBILITY.

DUE TO THE EXTENT OF DAMAGE THIS IS CONSIDERED TO BE AN ACCIDENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR21LA168](https://openairdocs.org/ntsb/WPR21LA168), Apr 20, 2021: Davis Donald M Bushby Mustang II (N284ND), Maricopa, AZ, substantial damage. Probable cause: The pilot’s failure to maintain airplane control during takeoff with a quartering right tailwind.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
