## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JUNE 3RD, 2021, AT APPROXIMATELY 12:15 CDT (1715 UTC), N41G, A SCHWEIZER (HUGHES) TH55A/269A ROTORCRAFT, WAS AIR TAXIING AT THE MONROE REGIONAL AIRPORT (KMLU) AND CRASHED IN THE GRASS BETWEEN THE TAXIWAY AND RAMP AREA.

N41G, A SMALL TWO SEAT HELICOPTER WAS AIR TAXIING AFTER LANDING AT APPROXIMATELY 20-30 FEET ABOVE THE TAXIWAY WHEN THE ROTORCRAFT SEEMINGLY LOST TAIL ROTOR EFFECTIVENESS. THE HELICOPTER BEGAN TO SPIN AND THE PILOT ATTEMPTED TO LAND IT IN THE GRASS, RESULTING IN AIRCRAFT DAMAGE. THE HELICOPTER CAME TO REST IN THE GRASS BETWEEN TAXIWAY ?G?, ?C? AND THE AVFLIGHT (FBO) RAMP. THE PILOT DID NOT HAVE TIME TO DECLARE AN EMERGENCY, SO ONLY AIRPORT FIRE, SECURITY, AND MAINTENANCE RESPONDED TO THE INCIDENT.

THERE WERE TWO SOLES ONBOARD. ONE INDIVIDUAL WAS THE PILOT AND THE SECOND WAS A PASSENGER. THERE WERE NO INJURIES REPORTED BY EITHER PERSON.

THE AIRCRAFT WAS RECOVERED FROM THE MLU TAXIWAY/RAMP AREA AND MOVED TO A NEARBY HANGAR #9. THE HELICOPTER IS IN A SECURE FACILITY AT THIS TIME.

THE AIRPORT MANAGER REPORTED THERE WAS NO DAMAGE TO THE TAXIWAY AND/OR AIRPORT PROPERTY.

OPERATOR ACCIDENT/INCIDENT HISTORY, SAFETY RECORD: A PILOT REVIEW WAS CONDUCTED UTILIZING MSAT-A ACCIDENT/INCIDENT DATA SYSTEM (AIDS) AND ENFORCEMENT INFORMATION SYSTEM (EIS) DATABASES WITH NO NEGATIVE FINDINGS. A REVIEW OF N41G WAS CONDUCTED USING THE MSAT-B DATABASE FOR ACCIDENT AND INCIDENT HISTORIES SHOWED A SIMILAR INCIDENT IN 2014. THE INCIDENT IDENTIFIED WAS WITH A DIFFERENT PILOT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN21LA253](https://openairdocs.org/ntsb/CEN21LA253), Jun 3, 2021: Schweizer TH55A/269A (N41G), Monroe, LA, substantial damage. Probable cause: The pilot’s failure to maintain yaw control during landing which resulted in a loss of tail rotor effectiveness.
