## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE PILOT OF THE HELICOPTER HAD JUST FINISHED A 14 CFR 137 AERIAL APPLICATION AND APPROACHED THE RE-SUPPLY TRUCK; LANDING FROM THE WEST (HEADING EAST). UPON TOUCHDOWN, THE PILOT STATED HE FELT "SOMETHING WASN'T RIGHT." THE PILOT STATED HE FELT A VERY SLIGHT REARWARD TIPPING SENSATION AND APPLIED SLIGHT FORWARD CONTROL INPUT. THE PILOT STATED THE HELICOPTER BEGAN TO YAW TO THE LEFT AND SLID OFF THE RE-SUPPLY TRUCK LANDING PAD. AN EYEWITNESS ON THE GROUND STATED THE HELICOPTER'S RIGHT SKID IMPACTED A 4-INCH RE-SUPPLY HOSE, AFTER YAWING, AND BENT IT'S BRACKET BACKWARDS. THE RE-SUPPLY HOSE WAS UNDAMAGED. AT THIS POINT, THE PILOT STATED THE HELICOPTER PITCHED FORWARD, CONTINUING A YAW TO THE LEFT. THE PILOT STATED THE HELICOPTER TWISTED AND ROLLED OFF THE TRUCK, LANDING ON ITS LEFT SIDE. THE HELICOPTER'S MAIN ROTOR HIT THE GROUND AS A RESULT, DESTROYING THE AIRCRAFT. THE PILOT STATED HE SHUT DOWN ALL SWITCHES AND CLIMBED OUT OF THE HELICOPTER. THE PILOT SUSTAINED NO INJURIES. DURING THE INVESTIGATION, NO AIRCRAFT ANOMALIES WERE FOUND. ALL CONTROL INPUTS MADE BY THE PILOT WERE IN ACCORDANCE WITH THE MANUFACTURER'S LIMITATIONS. THE ACCIDENT INVESTIGATION WAS CLOSED WITH NO FURTHER ACTION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN21LA432](https://openairdocs.org/ntsb/CEN21LA432), Jul 9, 2021: Bell 206L-3 (N170AM), Berlin, IL, substantial damage. Probable cause: The pilot’s failure to maintain control of the helicopter with a stuck skid, which resulted in a dynamic rollover.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
