## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JULY 17, 2021, AN INCIDENT REPORT WAS GENERATED FOR CIVIL AIRCRAFT N804UP, A TEXTRON AVIATION, INC. MODEL BE-300-B300 WITH SERIAL NUMBER FL-866 ^PRIVACY DATA OMITTED^ THE REPORT STATED THAT AT APPROXIMATELY 1410 EASTERN STANDARD TIME, ?SHORTLY AFTER DEPARTURE FROM CHARLEVOIX MUNICIPAL AIRPORT (KCVX), LOCATED IN CHARLEVOIX, MICHIGAN, THE LEFT ENGINE COWLING FELL OFF INTO LAKE MICHIGAN.? TWO CREW MEMBERS WERE ON BOARD AND NO INJURIES WERE REPORTED.

THE CREW STATED THAT AFTER DEPARTING RUNWAY TWENTY SEVEN (27) AT APPROXIMATELY THREE THOUSAND (3000) FEET ABOVE GROUND LEVEL, AS THE AIRCRAFT TURNED FOR THEIR WAYPOINT ?LYSTER?, THE CAPTAIN OBSERVED THE LEFT ENGINE OUTBOARD ACCESS DOOR HAD OPENED, STARTED FLUTTERING, DEPARTED THE AIRCRAFT AND FELL INTO LAKE MICHIGAN. THE AIRCRAFT RETURNED TO KCVX AND TAXIED TO THE RAMP WITHOUT FURTHER INCIDENT. THE CERTIFICATE HOLDER WAS NOT ABLE TO RECOVER THE DEPARTED ENGINE COWL DOOR FOR INSPECTION TO DETERMINE THE ROOT CAUSE.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
