## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON SEPTEMBER 28, 2021, N212HT, AN R44 LANDED ON ALPHA HELIPAD ON IFC NORTH RAMP, MIAMI EXECUTIVE AIRPORT. WHILE MANEUVERING TO THE RAMP AND LANDING, THE PILOT INDUCED A LOSS OF YAW CONTROL. LIFTED AND CRASHED INTO THE FENCE. IT IS EVIDENT, THE PILOT DID NOT ROLL THE THROTTLE DOWN FROM 101% WHEN HE LANDED. THE AGGRESSIVENESS OF THE LEFT YAW REQUIRES FULL LEFT PEDAL INPUT WITH THE ENGINES PRODUCING MORE THAN IDLE SPEED. WITH THE EASE AND SPEED OF THE HELICOPTER LIFTING, REQUIRED A LIFTING OF THE COLLECTIVE AND ENGINES PRODUCING 101%. MIAMI DADE FIRE RESCUE CALLED THE TOWER TO ADVISE ABOUT THE HELICOPTER, AND AN ALERT III WAS ISSUED. 4 SOB, MINOR INJURIES REPORTED BY ^PRIVACY DATA OMITTED^

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA21LA387](https://openairdocs.org/ntsb/ERA21LA387), Sep 28, 2021: Robinson Helicopter R44 II (N212HT), Miami, FL, substantial damage. Probable cause: The pilot’s loss of helicopter control after landing. Also causal was the pilot’s intentional operation of the helicopter without the required training, experience, and endorsements.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
