## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON OCTOBER 12, 2021, AT APPROXIMATELY 09:34 CDT AN EXPERIMENTAL AMATEUR BUILT GLASTAR AIRCRAFT WAS TRAVELING FROM MILWAUKEE, WI TIMMERMAN AIRPORT (MWC) TO THE SALINA, KS AIRPORT (SLN) UNDER A 14 CFR PART 91 FLIGHT WITH THE PILOT AND ONE PASSENGER ON BOARD. THE AIRCRAFT DEPARTED THE MWC AIRPORT UNDER A SPECIAL VISUAL FLIGHT RULES (SVFR) CLEARANCE AND PROCEEDED UNDER VISUAL FLIGHT RULES (VFR). WHILE EN ROUTE, THE AIRCRAFT DESCENDED IMPACTING A TREE. THE AIRCRAFT THEN PROCEEDED ACROSS A ROAD WHERE IT IMPACTED A SET OF TREES, AND THE GROUND WHEREUPON THE AIRCRAFT BROKE INTO SEVERAL PARTS CREATING A DEBRIS FIELD THAT EXTENDED APPROXIMATELY 200 FEET INTO A WOODED AREA. THE PILOT AND PASSENGER SUSTAINED FATAL INJURIES. INSTRUMENT METEOROLOGICAL CONDITIONS PREVAILED AT THE TIME OF THE ACCIDENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN22FA013](https://openairdocs.org/ntsb/CEN22FA013), Oct 12, 2021: New Glasair Glastar (N628RS), Waukesha, WI, destroyed, 2 fatal. Probable cause: The noninstrument-rated pilot’s decision to depart and continue flight into instrument meteorological conditions, which resulted in a loss of control due to spatial disorientation.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
