CUTAWAY WITH LOW AND NO RESERVE DEPLOYMENT. ON JANUARY 15, 2022, AT AIRGALDES AIRPARK (H21S), CLEWISTON, FLORIDA,^PRIVACY DATA OMITTED^ CONDUCTED NORMAL SKYDIVING FROM A CE 208 AIRCRAFT. ONE FEMALE JUMPER WITH SEVERAL YEARS OF EXPERIENCED ENCOUNTERED A HARD OPENNING. THE HARD OPENING OF HER PARACHUTE MAY HAVE RENDERED HER TEMPORARY UNCONSCIOUS. SHE ENCOUNTERED A SPIN AFTER HER MAIN PARACHUTE WAS FULLY DEPLOYED. PARTICIPANTS/GROUND WITNESSES NOTED HER CUTAWAY AT TOO LOW ALTITUDE FOR HER RESERVE (EMERGENCY) CHUTE TO OPEN. HER PARACHUTE SYSTEM WAS NOT EQUIPPED WITH A RESERVE STATIC LINE (HOOK) OR AUTOMATIC ACTIVATION DEVICE. ONCE SHE CUTAWAY HER MAIN PARACHUTE, HER FATAL FREE FALL WAS SECONDS TO FINAL GROUND IMPACT. POST ACCIDENT HER MAIN CHUTE, AND RESERVE WAS CHECKED AT ^PRIVACY DATA OMITTED^, PRESIDENT^PRIVACY DATA OMITTED^. ^PRIVACY DATA OMITTED^ NO SIGNIFICANT DEFECTS WITH HER PARACHUTE GEAR. HER RESERVE CHUTE REMAINED FULLY PACKED, AND WAS NEVER DEPLOYED.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
