## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

2022-01-29-N88140-INCIDENT-WENT OFF THE LEFT SIDE OF RUNWAY 30 AT AIRLAKE (KLVN). PIC RECENTLY OBTAINED TAIL WHEEL ENDORSEMENT (18.4 HOURS TAILWHEEL TIME) LOST CONTROL OF AIRCRAFT ON TAKEOFF. THE TAILWHEEL STARTED TO LIFT OF AND BOUNCE, THE PIC FAILED TO FLY THE TAIL AND A LIGHT CROSSWIND TURNED THE AIRCRAFT TO THE LEFT. THE LEFT MAIN TIRE HIT A SNOW BANK CAUSING IT TO TURN HARD LEFT AND TIP UP ON ITS NOSE AND THE PROPELLER CONTACTED THE GROUND. NO INJURIES AND DAMAGE LIMITED TO PROP STRIKE AND MINOR DAMAGE TO LOWER COWLING. AIRCRAFT RAMP AND RECORDS CHECK FOUND SEVERAL ISSUES. EXPIRED REGISTRATION, NO PROPELLER TOTAL TIME LISTED IN ANNUAL INSPECTION, AD?S NOT PROPERLY LOGGED, MISSING METHOD OF COMPLIANCE, AD REVISION DATE AND TIME AND DATE WHEN NEXT ACTION IS REQUIRED FOR RECURRING AD?S, ELT INSPECTION, UNABLE TO LOCATE AIRCRAFT DATA PLATE. ALL ISSUES HAVE BEEN ADDRESSED WITH AIRCRAFT OWNER AND WILL BE CORRECTED PRIOR TO FURTHER FLIGHT. COUNSELING WAS COMPLETED WITH THE PILOT AND THEY ARE AWARE OF DEFICIENCIES. PILOT AGREED TO OBTAIN ADDITIONAL TRAINING.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
