## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AIRCRAFT WAS LANDING ON RWY 15. AIRCRAFT DEPARTED THE RUNWAY TO THE LEFT. BOUNCED APPROXIMATELY 3 TIMES THRU THE GRASS. AIRCRAFT THEN HIT AN EARTHEN BERM AT THE FAR EDGE OF THE GRASS AND BECAME AIRBORNE. THE TERRAIN SLOPED AWAY FROM THE BERM. AIRCRAFT TOUCHED BACK DOWN ON THE LEFT WING TIP, AND IN A CARTWHEEL MOTION, IMPACTED THE NOSE/PROPELLER ASSEMBLY, RIGHT WING TIP AND EMPENNAGE. THE AIRCRAFT THEN FELL FORWARD AND CAME TO REST ON THE LANDING GEAR, WITH THE FRONT GEAR COLLAPSED OR FOLDED OVER. FLIGHT CONTROL CONTINUITY WAS VERIFIED AT THE ACCIDENT SITE. PROPELLER DAMAGE INCLUDED FORWARD BENDING.

THE PILOT BELIEVES THE FRONT LANDING GEAR FAILED AND CAUSED THE DEPARTURE TO THE LEFT FROM THE RUNWAY. NTSB WANTS TO EXAMINE THE FRONT LANDING GEAR, BY VISUAL INSPECTION AND NON-DESTRUCTIVE TESTING (NDT) MEANS.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN22LA191](https://openairdocs.org/ntsb/CEN22LA191), May 4, 2022: Vans RV6A (N5060), Mankato, MN, substantial damage. Probable cause: The pilot’s failure to maintain directional control while landing with a higher airspeed.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
