## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE AIRCREW OPERATING PUBLIC USE AIRCRAFT WERE PERFORMING PRACTICE AUTOROTATIONS AT NIGHT. THE PILOT HAD COMPLETED TWO STRAIGHT IN AUTOROTATIONS AND ONE FROM 90 DEGREES. ALL AUTORATIONS WERE AIRBORNE RECOVERIES.

ON THE THIRD AUTOROTATION THE INSTRUCTOR PILOT STATED HE RETURNED THE THROTTLE TO FLIGHT IDLE AT 400 FEET. THE PILOT UNDER INSTRUCTION CONFIRMED STATED HE FELT THE INSTRUCTOR PILOT ENGAGE THE THROTTLE.

THE AIRCRAFT CONTINUED TO DESCEND. AT APPROXIMATELY 150 TO 100 FEET THE PILOT UNDER INSTRUCTION FLARED THE AUTOROTATION. THE AIRCRAFT CONTINUED TO SETTLE AND HIT THE RUNWAY AND SLID FORWARD. THE AIRCREW REPORTED HEARING THE ENGINE SPOOL UP AND FELT THE AIRCRAFT YAW AFTER IMPACTING THE RUNWAY.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN22LA402](https://openairdocs.org/ntsb/CEN22LA402), Aug 30, 2022: MD Helicopters 369E (N1576F), Pearland, TX, substantial damage. Probable cause: The pilot receiving instruction’s failure to maintain aircraft control, and the flight instructor’s delayed remedial action, which resulted in a hard landing.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
