## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON SATURDAY, OCTOBER 1, 2022, AT APPROXIMATELY 1100 PACIFIC DAYLIGHT TIME (PDT), A BELL 206B JETRANGER WHO WAS ON A “RETURN TO SERVICE” MAINTENANCE FLIGHT, WAS INVOLVED IN AN ACCIDENT WHEN THE AIRCRAFT WHO JUST HAD DEPARTED FROM THE FRESNO YOSEMITE INTERNATIONAL AIRPORT (KFAT) LOST TAIL ROTOR EFFECTIVENESS AND STARTED A SPIRALING DECENT, FOLLOWED BY AN IMPACT NEXT TO A HOME. THE ACCIDENT OCCURRED APPROXIMATELY 4.5 MILES SOUTH OF KFAT. THE AIRCRAFT WAS DESTROYED UPON IMPACT. THE PILOT WAS SERIOUSLY INJURED, AND THE PASSENGER HAD MINOR INJURIES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR23LA004](https://openairdocs.org/ntsb/WPR23LA004), Oct 1, 2022: Bell 206B (N284CA), Fresno, CA, substantial damage. Probable cause: The disbond of a tail rotor drive shaft splined adapter, which resulted in a loss of tail rotor drive and a subsequent hard landing.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
