## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON OCTOBER 26, 2022, PIPER PA-22-160, ^PRIVACY DATA OMITTED^WAS FLYING VFR FROM HARRISON COUNTY (KASL @ MARTIAL TX) - FORT SMITH REGIONAL AIRPORT (KFSM) - BOONE COUNTY AIRPORT (KHRO @ HARRISON, AR). THIS WAS AN ANGEL FLIGHT FROM KASL-KFSM. THE PILOT CLAIMS HE MAY/MAY NOT HAVE COMPLETELY TOPPED OFF TO 36 GALLONS AT KASL AND DEPARTED FOR A PLANNED 2.5 HOUR FLIGHT. MEMPHIS CENTER CANCELED RADAR SERVICES WITH THE AIRPLANE ABOUT 1-2 MINUTES PRIOR TO FUEL EXHAUSTION ABOUT 15 MILES PRIOR TO THE DESTINATION AIRPORT.

^PRIVACY DATA OMITTED^ STARTED THE DECENT INTO KHRO AND THEN THE AIRCRAFT APPEARED TO RUN OUT OF FUEL CAUSING A COMPLETE POWER FAILURE. THERE WAS NO MECHANICAL REASON FOUND FOR THE FUEL STARVATION, IT WAS DETERMINED THAT IT WAS POSSIBLY NOT COMPLETELY TOPPED OFF AS THOUGHT. THIS COUPLED WITH HEADWINDS, AT APPROXIMATELY 1525 CENTRAL TIME^PRIVACY DATA OMITTED^LANDED IN A FIELD, WHILE ROLLING OUT, AFTER A SUCCESSFUL LANDING, THE NOSE GEAR HIT A HOLE, COLLAPSING THE NOSE GEAR SPINNING THE AIRCRAFT AROUND AND COMING TO REST AT A LAT/LONG OF APPROXIMATELY N36.16, W93.35 WHICH IS ABOUT 12NM SW OF KHRO, CAUSING SUBSTANTIAL DAMAGE TO THE AIRCRAFT, NO INJURIES TO THE SINGLE OCCUPANT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN23LA020](https://openairdocs.org/ntsb/CEN23LA020), Oct 26, 2022: Piper PA-22-160 (N8457D), Osage, AR, substantial damage. Probable cause: A total loss of engine power due to fuel exhaustion. Contributing to the accident was the pilot’s inadequate preflight inspection.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
