## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

OUR OFFICE WAS NOTIFIED ON JANUARY 2, 2023 OF A FATAL ACCIDENT THAT OCCURRED ON JANUARY 2, 2023 AT ABOUT 11:35 LOCAL TIME, OF ^PRIVACY DATA OMITTED^THAT WAS SUBSTANTIALLY DAMAGED AT PROVO MUNICIPAL AIRPORT, PROVO, UTAH. THE WEATHER CONDITIONS WERE THAT IT WAS SNOWING AT THE TIME OF THE ACCIDENT. THE PILOT REMOVED THE AIRCRAFT FROM HIS HANGER AT 10:55 LOCAL TIME. THE PILOT TAXIED OVER TO A LOCAL FBO AND GOT 350 GALLONS OF JET-A FUEL 5 MINUTES LATER. THE ENGINES STARTED BETWEEN 11:10 OR 11:15 WHICH IS WHEN THE SNOW BEGAN TO FALL.

WITNESSES STATED THAT THE AIRCRAFT WAS ON DEPARTURE ROLL AND APPEARED TO PULL UP STEEP AND ROLL TO THE LEFT. ANOTHER WITNESS STATED THAT THE AIRCRAFT ASCENDED TO ABOUT 20-30 FEET ABOVE THE GROUND.

THE AIRCRAFT IMPACTED THE RUNWAY SURFACE AT ABOUT 2,626 FEET FROM THE APPROACH END OF RUNWAY 13 AND 20 FEET LEFT OF THE CENTERLINE. THE SCRAP MARK EXTENDED ABOUT 91 FEET AND WAS PARALLEL TO THE RUNWAY CENTERLINE.

A SECOND SCRAPE MARK WAS OBSERVED ABOUT 2,903 FT FROM THE APPROACH END OF RUNWAY 13, AND ARCED LEFT TO THE LEFT EDGE OF THE RUNWAY SURFACE ABOUT 3,126 FT FROM THE APPROACH END OF THE RUNWAY. A SWATH OF DISPLACED SNOW AND DIRT EXTENDED FROM THE RUNWAY’S LEFT EDGE ABOUT 100 FT TO A LARGE IMPACT CRATER. THE CRATER CONTAINED PORTIONS OF AIRFRAME DEBRIS AND VARIOUS AIRFRAME COMPONENTS. WRECKAGE DEBRIS EXTENDED FROM THE CRATER ABOUT 597 FEET TO THE FUSELAGE. THE AIRCRAFT WINGS WERE SEPARATED FROM THE AIRFRAME AND LOCATED ABOUT 106 FEET BEYOND THE FUSELAGE. BOTH ENGINES WERE SEPARATED AND LOCATED ADJACENT TO THE FUSELAGE.

THE PILOT SUSTAINED FATAL INJURIES, TWO PASSENGER SUSTAINED SERIOUS INJURIES, AND PASSENGER SUSTAINED MINOR INJURIES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR23FA080](https://openairdocs.org/ntsb/WPR23FA080), Jan 2, 2023: Embraer Executive EMB-505 (N555NR), Provo, UT, substantial damage, 1 fatal. Probable cause: The pilot’s failure to deice the airplane before takeoff in weather conditions conducive to ice accumulation, which resulted in an ice-contaminated wing and subsequent stall during takeoff.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
