## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON FEBRUARY 23, 2023 AT APPROXIMATELY 11:48 EASTERN TIME, THE PILOT ROTATED AT 45-50 MPH AND NEVER WENT ABOVE 40 MPH WHILE ATTEMPTING TO TAKEOFF ON RUNWAY 17 AT TWELVE OAKS PRIVATE AIRFIELD (II87) IN A PIPER PA-28 ^PRIVACY DATA OMITTED^. IN HER ATTEMPT TO CLIMB, THE AIRCRAFT SETTLED BACK TO THE GROUND AND THE PILOT ROTATED A SECOND TIME. THE AIRSPEED OF THE AIRCRAFT WOULD NOT INCREASE DUE TO THE SLOW SPEED OF APPROXIMATELY 40 MPH. FEARING A STALL, THE PILOT RELEASED BACK PRESSURE ON THE YOKE AND PUSHED THE AIRCRAFT NOSE DOWN TO ATTEMPT TO LAND. THE AIRCRAFT IMPACTED THE GROUND AND STRUCK A BUSH ON THE RIGHT WING, SPINNING THE AIRCRAFT 180 DEGREES. THE AIRCRAFT THEN SLID DOWN A HILL AND CAME TO A STOP APPROXIMATELY 50 YARDS OFF THE END OF THE RUNWAY. THE AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE. THE EARLY ROTATION WAS BELOW STALL SPEED AND CREATED A LOSS OF CONTROL SITUATION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN23LA119](https://openairdocs.org/ntsb/CEN23LA119), Feb 23, 2023: Piper PA-28-140 (N1700T), Martinsville, IN, substantial damage. Probable cause: The pilot’s inadequate weather evaluation and failure to abort the takeoff that resulted in an impact with terrain.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
