## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AS PILOT PULLED COLLECTIVE PITCH, NOT REALIZING THE AMOUNT OF LEFT TORQUE PEDAL INPUT HE HAD APPLIED, THE AIRCRAFTS WEIGHT ON SKIDS DECREASED, THE AIRCRAFT IMMEDIATELY STARTED ROTATING TO THE LEFT. THE INITIAL CONTACT WITH THE GROUND WAS THE RIGHT HORIZONTAL STABILIZER TRAILING EDGE WHICH STARTED THE SEQUENCE OF THE TAIL BOOM/TAIL ROTOR DEPARTING THE AIRCRAFT. AIRCRAFT CAME TO REST INVERTED APPROXIMATELY 320 DEGREES FROM THE TAKEOFF POINT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR23LA214](https://openairdocs.org/ntsb/WPR23LA214), Jun 3, 2023: Bell 206B (N205PA), Filer, ID, substantial damage. Probable cause: The pilot’s improper anti-torque pedal inputs during takeoff, which resulted in a loss of helicopter control during takeoff and subsequent dynamic rollover.

## Other reports on N205PA (1)

Matched to the same aircraft by N-number and date.

- [Incident 20120818020439I](https://openairdocs.org/incident/20120818020439I), Aug 18, 2012: Bell 206B (N205PA), Point Hope, AK, Substantial damage. Suffered engine failure 50 ft above ground and at 40 knots on short final entered autorotation and landed hard.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
