## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

TWO INDEPENDENT WITNESSES STATED THE PILOT STARTED THE TAKEOFF ROLL WITH THE ROTOR IN A HORIZONTAL POSITION, INSTEAD OF PITCHED BACK AS REQUIRED FOR THE ROTOR TO STORE ENERGY FOR FLIGHT. THE WITNESSES BOTH STATED THE TAKEOFF ROLL WAS MUCH LONGER THAN USUAL (1000+ FEET), AND WHEN THE PILOT PITCHED THE ROTOR UP, THE AIRCRAFT PITCHED UP, GAINED ABOUT 30 TO 40 FEET ABOVE GROUND, ROLLED TO THE LEFT, AND IMPACTED THE RUNWAY. THE LEFT MAIN WHEEL WAS BROKEN OFF AT THE STRUT MOUNT. THE AIRCRAFT CONTINUED TO ROLL TO THE LEFT AND THE ROTORS CONTACTED THE GROUND. THE FUSELAGE DEPARTED THE RUNWAY ON ITS LEFT SIDE, TRAPPING THE PILOT’S LEFT ARM UNDER THE REAR WINDSHIELD. THE AIRCRAFT CAME TO REST 1400 FEET FROM THE DEPARTURE POINT OF THE RUNWAY.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN23LA386](https://openairdocs.org/ntsb/CEN23LA386), Aug 20, 2023: Silverlight AR-1 (N57AR), Granbury, TX, substantial damage. Probable cause: The pilot’s improper takeoff procedure which resulted in a loss of control during takeoff.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
