## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON AUGUST 25, 2023, AT APPROXIMATELY 1009MDT A CESSNA 195B, SINGLE ENGINE AIRPLANE, N2192C, ^PRIVACY DATA OMITTED^, WAS INVOLVED IN AN ACCIDENT. UPON THE TAKEOFF ROLL, THE PILOT LOST CONTROL AND DEPARTED RUNWAY 36 TO THE LEFT AT LOS LUNAS MIDVALLEY AIRPORT (E98) AND THE AIRCRAFT CAME TO REST UPSIDE DOWN ON THE LEFT SIDE OF THE RUNWAY 36. NO FLIGHT PLAN WAS FILED. THE FLIGHT ORIGINATED FROM E98 AND WAS THE FIRST FLIGHT OF THE DAY. THE AIRCRAFT SUSTAINED SUBSTANTIAL DAMAGE. THE PILOT WAS ON A PERSONAL, LOCAL FLIGHT FROM E98, WHERE THE AIRCRAFT IS BASED. THE ATP PILOT INCURRED MINOR INJURIES.

THE PILOT STATED "AT SOMETIME DURING RUN UP I THOUGHT I COULD MOVE THE SEAT BACK ONE MORE NOTCH. IT FELT AS IF I HAD AND PINS WERE IN THE HOLES". HE STATED THAT HE ADDED POWER FOR TAKEOFF AND AS THE AIRCRAFT ACCELERATED THE SEAT CAME BACK AND HE COULD BARELY REACH THE RUDDER PEDALS. HE REDUCED POWER TO IDLE AND LOST CONTROL AS THE AIRCRAFT DEPARTED THE RUNWAT TO THE LEFT COMING TO REST UPSIDE DOWN.

THE PILOT STATED THAT HE HAD REMOVED THE COTTER PIN SEAT STOPS AND DID NOT REPLACE THEM BEFORE THE FLIGHT THUS CAUSING THE REAR SEAT ROLLERS TO COME OFF THE TRACKS WHEN THE AIRCRAFT ACCELERATED FOR TAKE-OFF.

THE ABQ WEATHER 251552Z 19008KT FEW040 FEW 250 25/15 A3028

THE PILOT NOT INSTALLING SEAT STOPS PRIOR TO FLIGHT, AND NOT ENSURING THE SEAT PINS WERE IN THE HOLES BEFORE TAKEOFF, CAUSING THE SEAT TO SLIDE BACK ON THE TAKEOFF ROLL ALLOWED THE AIRCRAFT TO BECOME UNCONTROLLABLE.

COMPLIANCE ACTION - COUNCELING WAS USED ON SEPTEMBER 28, 2023. DETAILS ARE RECORDED IN SAS TASK T-SW01-FY23-0714.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR23LA337](https://openairdocs.org/ntsb/WPR23LA337), Aug 25, 2023: Cessna 195B (N2192C), Los Lunas, NM, substantial damage. Probable cause: The pilot’s failure to secure the seat’s position in the seat rail, which resulted in a loss of directional control and nose over.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
