## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

FOLLOWING TAKEOFF FROM HIO THE MULTI-ENG CFI AND TRAINING COMMERCIAL PILOT APPEARED TO HAVE PERFORMED STALL TRAINING AND STEEP TURNS WITH A PRIVATE PILOT AS PASSENGER/OBSERVER ABOARD. THE COMMERCIAL AND PRIVATE PILOTS WERE BEGINNING MULTI-COMMERCIAL TRAINING. AT ABOUT 20 MINUTES INTO THE FLIGHT, DURING AN APPARENT VMC STALL DEMONSTRATION, THE AIRPLANE APPEARED TO SLOW TO ABOUT 50 KNOTS AND THEN WENT INTO A SPIN AT ABOUT 5100’. THE AIRPLANE WAS IN ABOUT AN 80 DEGREE NOSE DOWN ATTITUDE, COUNTER-CLOCKWISE SPIN WHEN IT IMPACTED THE ROOF OF A HOUSE RESULTING IN THE CFI AND COMMERCIAL PILOT BEING DECEASED AND THE PASSENGER RECEIVING SERIOUS INJURIES.

A PRELIMINARY STATEMENT FROM THE PASSENGER/OBSERVER STATED THAT PRIOR TO THE ACCIDENT: SLOW FLIGHT, STALLS, AND STEEP TURNS WERE PERFORMED WITH NO ISSUES. A VMC DEMO WAS THE LAST MANEUVER. THE PASSENGER/OBSERVER RECALLS THERE BEING MORE THAN 5 DEGREES OF BANK. THIS WAS HER FIRST TIME SEEING A VMC DEMO SO SHE ISN’T SURE IF THE MANEUVER WAS DONE CORRECTLY/INCORRECTLY AND IF THE RECOVERY PROCEDURES WERE DONE CORRECTLY/INCORRECTLY. THE PLANE STARTED GOING LEFT AND VERY QUICKLY BECAME INVERTED. SHE DOESN’T RECALL WHEN THE TRANSFER OF CONTROLS HAPPENED, BUT THE STUDENT WAS THE ONE THAT ASKED THE CFI TO TAKE CONTROLS. SHE COULDN’T SEE WHAT THE CFI WAS DOING SINCE SHE WAS SITTING BEHIND HIM. THE CFI AND STUDENT WERE CALMLY TRYING TO SOLVE THE PROBLEM ON THE WAY DOWN TALKING TO EACH OTHER THE WHOLE TIME. SHE CLOSED HER EYES AT THE LAST FEW SECONDS AND DOESN’T REMEMBER IT.
