## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE HELICOPTER PILOT TOUCHED DOWN TOO FAR AFT OVER THE CART/PAD DURING LANDING CAUSING THE AIRCRAFT TO SLIDE BACKWARDS OFF THE CART. THE PILOT THEN REACTED WITH FULL FORWARD CYCLIC AND INCREASED COLLECTIVE TO TRY TO REGAIN CONTROL OF THE AIRCRAFT AND GET THE AIRCRAFT AIRBORNE. THESE CONTROL INPUTS CAUSED THE AIRCRAFT TO BE THRUSTED BACK INTO THE AIR BUT WITH VERY LOW TAIL ROTOR INERTIA. THE HELICOPTER BEGAN TO CLIMB 50 FT WHILE ROTATING AND THEN FELL WHILE ROTATING TO THE GROUND IN AN UPRIGHT POSITION IMPACTING THE GROUND.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN24LA097](https://openairdocs.org/ntsb/CEN24LA097), Jan 21, 2024: Robinson Helicopter R22 Beta (N8350M), Kenosha, WI, substantial damage. Probable cause: The pilot’s failure to maintain control of the helicopter while landing.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
