## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON APRIL 1, 2023, A PARACHUTE JUMPERS WAS ENGAGED IN AN AERIAL FORMATION WITH A TEAM OF JUMPERS. TRAGICALLY, THE LEAD JUMPER OF THE TEAM PASSED AWAY DURING THE INCIDENT, AND ANOTHER MEMBER SUSTAINED MINOR INJURIES WITHOUT NEEDING MEDICAL ATTENTION AFTER A MIDAIR COLLISION. THE LEAD JUMPER HAD ALREADY DEPLOYED HIS PARACHUTE WHICH WAS FULLY DEPLOYED AT THE TIME OF THE COLLISION. SHORTLY BEFORE THE COLLISION, THE OTHER TEAM MEMBER REALIZED THAT A COLLISION WAS IMMINENT AND DEPLOYED HIS PARACHUTE IN TIME TO AVOID A DIRECT IMPACT. UNFORTUNATELY, THE ANGLE OF DESCENT CAUSED THIS TEAM MEMBER TO COLLIDE FROM ABOVE, COLLAPSING THE LEAD JUMPER'S PARACHUTE AND RENDERING THE LEAD JUMPER INCAPACITATED. LEAD JUMPER PARACHUTE REDEVELOPS AND WAS UNCONTROL. AS A RESULT, THE TEAM LEAD PLUMMETED TO THE GROUND HIT A BUILDING AND WAS FOUND UNRESPONSIVE.

INSPECTED PARACHUTE OF THE DECEASED JUMPER FOUND NO DEFECTS.

REVIEW OF JUMP TEAM VIDEO FROM THE ACCIDENT SHOWS DECEASED JUMP PARACHUTE OPERATING CORRECTLY.

## NTSB investigation

None matched to this report by aircraft and date. Search the NTSB's own records: https://data.ntsb.gov/carol-main-public/basic-search

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
