## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AT ~1730MST, THE AIRCRAFT DEPARTED U42 WITH THE INTENDED DESTINATION OF AN OFF-AIRPORT LANDING STRIP ON FREMONT ISLAND IN THE GREAT SALT LAKE. UPON ARRIVAL, THE PIC PASSED OVER THE WINDSOCK AT ~300’ AGL AND PERFORMED AN INSPECTION PASS OF THE STRIP. NO OBSERVATIONS OF WINDS OR VISIBLE OBSTACLES WERE NOTED ON THE LANDING STRIP. THE PIC CAME AROUND INTENDING TO LAND FROM THE NORTH FLYING DOWNWIND AT ~800’ AGL. WHILE TURNING INTO FINAL, THE PIC STATED THAT THEY VISUALLY CHECKED THAT THE PASSENGER WAS NOT ON THE CONTROLS; HOWEVER, THE PIC DID NOT PERFORM A VERBAL CHECK WITH THE PASSENGER. THE PIC’S APPROACH WAS AT ~55MPG WHILE ENTERING A FLARE AND SLOWED TO ~40-45 MPH LEADING INTO TOUCHDOWN. IMMEDIATELY DURING TOUCHDOWN, THE AIRCRAFT WENT NOSE OVER AND THE AIRCRAFT CAME TO REST INVERTED. NO PRIOR MAINTENANCE PERFORMED INDICATES CONTRIBUTING TECHNICAL FACTORS AND THE PIC STATED THERE WERE NO MECHANICAL INDICATORS TO NOTE DURING THE FLIGHT. NTSB INVESTIGATIONS TO DETERMINE CAUSE ARE ONGOING AND THIS REPORT WILL AMEND CORRECTIVES ACTIONS BASED ON THOSE INVESTIGATION RESULTS. COUNSELING IS BEING RECOMMENDED DUE TO THE PIC NOT MAINTAINING CONTROL OF THE AIRCRAFT DURING A CRITICAL PHASE OF FLIGHT. NO FURTHER ACTION IS REQUIRED AT THIS TIME.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR24LA168](https://openairdocs.org/ntsb/WPR24LA168), Jun 1, 2024: Maule M-5-210C (N62062), Hooper, UT, substantial damage. Probable cause: The inadvertent application of brakes during landing that resulted in a nose over.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
