## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT STATED THAT PRIOR TO FLIGHT, HE CHECKED THE WEATHER VIA TELEPHONE FROM JACKSONVILLE FLIGHT WEATHER AND PERFORMED A NORMAL PRE-FLIGHT INSPECTION OF THE AIRCRAFT. PILOT STATED THAT HE TAXIED TO RUNWAY 18, WHICH WAS ACTUALLY RUNWAY 36, AND BEGAN TAKE-OFF ROLL. ACCORDING TO EYEWITNESS STATEMENTS, ONCE THE AIRCRAFT BROKE CONTACT WITH THE SURFACE OF THE RUNWAY, THE AIRCRAFT WAS OUT OF CONTROL. AIRCRAFT VEERED TOWARDS THE TREES OVER THE WEST SIDE TAXIWAY THAT RUNS PARALLEL TO THE RUNWAY, AND AT ONE TIME AIRCRAFT WAS PERPENDICULAR TO THE RUNWAY. PILOT STATED THAT HE REMEMBERED A CROSSWIND AS SOON AS THE AIRCRAFT REACHED THE TOP OF THE TREELINE ON THE WEST SIDE AND ATTEMPTED TO CORRECT. AFTER THIS, PILOT HAS NO MEMORY OF FURTHER FLIGHT. WITNESS STATEMENTS THEN SAY THAT AIRCRAFT IMPACTED THE TREES ON THE EAST SIDE OF THE RUNWAY, BOUNCED ONCE, WITH IMPACT MARKS VERIFYING THIS. AIRCRAFT THEN IMPACTED RUNWAY AND FLIPPED UPSIDE DOWN WHERE IT CAME TO REST. NEIGHBORS CAME TO THE PILOTS AID AND REMOVED HIM FROM THE AIRCRAFT WRECKAGE DUE TO FIRE CONCERNS AND WAITED FOR EMS TO ARRIVE. SUBSEQUENT INVESTIGATION REVEALED THAT PILOT HAD MEDICAL CERTFICATE DENIED IN 2011.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA24LA243](https://openairdocs.org/ntsb/ERA24LA243), Jun 2, 2024: Eide Turner T-40A (N4DE), Middleburg, FL, substantial damage. Probable cause: The pilot’s loss of control during climb out, which resulted in a collision with a tree.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
