## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

TRAINING FLIGHT DEPARTED FROM KHUM (HOUMA) ABOUT 9:30 AM IN A CESSNA 140. THE PILOT AT THE CONTROLS WAS IN THE LEFT SEAT AND THE INSTRUCTOR WAS IN THE RIGHT SEAT. THEY FLEW TO L83 (THIBODAUX) TO PRACTICE TAKE-OFFS AND LANDINGS. THEY COMPLETED 2 LANDINGS TO A FULL STOP WITHOUT INCIDENT. ON THE NEXT TAKE-OFF ROLL THE PILOT OVER-CONTROLLED FOR RUNWAY ALIGNMENT AND, IN THE PROCESS, PUT ON BREAKS WHICH CAUSED THE NOSE-OVER. NEITHER OCCUPANT WAS INJURED IN THE ACCIDENT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN24LA237](https://openairdocs.org/ntsb/CEN24LA237), Jun 22, 2024: Cessna 140 (N77143), Schriever, LA, substantial damage. Probable cause: The pilot’s inadvertent application of the toe brakes during the takeoff roll.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
