As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT REPORTED LOSS OF POWER AND ENGINE FAILURE AT 200 FEET DURING TAKEOFF. PILOT CHOOSE TO TRY AND MAKE THE REMAINING RUNWAY AND LAND BACK. THIS RESULTED IN A HARD LANDING AND SUBSTANTIAL DAMAGE TO AIRCRAFT.

DURING THE INSPECTION OF THE AIRCRAFT THE GASCOLATOR WAS FOUND TO BE CANTED TO ONE SIDE AND NO SAFETY WIRE WAS FOUND ON THE BAILS OR THE JAM NUT THAT SECURES THE GASCOLATOR IN PLACE. THERE WAS ALSO AN INDICATION OF A FUEL LEAK ON THE BOTTOM OF THE AIRCRAFT NEAR THE GASCOLATOR.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN24LA268](https://openairdocs.org/ntsb/CEN24LA268), Jul 16, 2024: Piper PA-28-181 (N1941V), Arcola, TX, substantial damage. Probable cause: Loss of engine power due to fuel starvation as a result of the mechanic’s failure to properly secure the fuel system gascolator strainer bowl following an annual inspection.

## Other reports on N1941V (1)

Matched to the same aircraft by N-number and date.

- [Incident 19880801052879I](https://openairdocs.org/incident/19880801052879I), Aug 1, 1988: Piper PA28181 (N1941V), Oshkosh, WI, Minor damage. Conflicting instructions from ATC caused pilot to land long. Overshot runway and came to rest in a ditch.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
