## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE CFI AND STUDENT PILOT (SP) WAS CLEARED FOR TAKEOFF AT KHIO RUNWAY 31L AT APPROXIMATELY 1555 PST IN ^PRIVACY DATA OMITTED^, CE-152. THE SP WAS PERFORMING THE TAKEOFF WHILE THE CFI WAS MONITORING.

THE SP SLOWLY ADDED POWER AND THE TAKEOFF WAS INITIATED. THE CFI HAD THE CONTROLS GUARDED. APPROX 2-3 SECONDS AFTER THE POWER WAS ADDED THE CFI SIDE DOOR OPENED. THE CFI MOMENTARILY GLANCED TO LOOK OVER AT THE DOOR WHEN THE SP DID NOT LET THE NOSE DOWN ENOUGH AND THE TAIL SKID STRIKE OCCURRED. BOTH CFI AND SP NOTICED THE TAILSTRIKE IMMEDIATELY AND THE CFI TOOK CONTROLS AND ABORTED THE TAKEOFF. KHIO TOWER WAS CONTACTED AND NOTIFIED OF THE ABORTED TAKEOFF AND THE AIRCRAFT SUCCESSFULLY TAXIED OFF OF THE RUNWAY AND BACK TO THE SCHOOL.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
