## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

THE COMMERCIAL PILOT WHILE PRACTICING SOFT FIELD TAKEOFFS FAILED TO PUT THE FLAPS IN THE CORRECT POSITION. AS A RESULT, THE AIRCRAFT LIFTED OFF THE RUNWAY EARLY. THE PILOT WAS UNABLE TO MAINTAIN DIRECTIONAL CONTROL AND THE RIGHT WING STRUCK THE PRECISION APPROACH PATH INDICATOR (PAPI). THE DAMAGE TO THE AIRCRAFT WAS MAJOR BECAUSE THE WING SEPARATED FROM THE FUSELAGE MOUNT. THE PILOT AND PASSENGER DID NOT SUSTAIN INJURIES.

AIRCRAFT RECORDS WERE REVIEWED AND FOUND TO HAVE PROPER AIRCRAFT CERTIFICATION AND CURRENT INSPECTION STATUS. PILOT RECORDS WERE REVIEWED AND FOUND TO HAVE THE CORRECT CERTIFICATION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR24LA322](https://openairdocs.org/ntsb/WPR24LA322), Sep 27, 2024: Vans RV-12 (N916PH), Paris, ID, substantial damage. Probable cause: The pilot’s failure to properly configure the flaps, which resulted in a loss of directional control during takeoff.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
