## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

AFTER PASSING THE FAF FOR THE ILS 10, SMOKE STARTED TO ENTER THE FLIGHT DECK

• BOTH PILOTS DONNED THEIR OXYGEN MASKS AND DECLARED AN EMERGENCY (MAYDAY, MAYDAY)

• UPON COMING TO A STOP ON A PARALLEL TAXIWAY, THE PILOTS COMPLETED THE EVACUATION CHECKLIST

• THE FIRST OFFICER EXITED THE FLIGHT DECK THROUGH THE WINDOW

• THE CAPTAIN OPENED THE L1 DOOR AND WAITED FOR THE FIRE DEPARTMENT TO BRING A LADDER TO IT

• THE FIRST OFFICER LANDED IN A PUDDLE OF HYDRAULIC FLUID

POST FLIGHT MAINTENANCE INSPECTION

• LOCAL MAINTENANCE IDENTIFIED A PIN HOLE LEAK IN HYDRAULIC PRESSURE SUPPLY TO NOSE LANDING GEAR INSIDE THE FORWARD FUSELAGE ELECTRONIC & EQUIPMENT BAY.

• THE PIN HOLE RESULTED IN VAPORIZED HYDRAULIC FLUID SPRAYING INTO E&E BAY DIRECTLY UNDER THE CAPTAIN SEAT AREA AND CONTAMINATING FLIGHT DECK.

• E/E BAY IS HEAVILY CONTAMINATED WITH HYDRAULIC FLUID, AND THERE IS ALSO SOME FLUID ON THE FLIGHT DECK FLOOR.

THIS AIRCRAFT FLEW BACK TO YVR FROM SJU ON NOV 16TH, 2024.

^PRIVACY DATA OMITTED^MAINTENANCE IS SATISFIED THAT THEY HAVE IDENTIFIED THE ROOT CAUSE OF THE SMOKE EVENT.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
