## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

STATMENT FROM THE PILOT:

THE PLANE NEEDED A LOT OF RIGHT RUDDER ON TAXIING FROM THE OLD TS TO RUNWAY 33 EVEN THOUGH THE WIND WAS FROM THE WEST. I TAXI'D OUT WITH T/O FLAPS SELECTED.

ON THE TAKEOFF ROLL, I HAD ALMOST FULL RIGHT RUDDER TO KEEP US FLYING PERFECTLY STRAIGHT ON TAKEOFF, I PEEKED AT THE WINDSOCK AND IT WAS ~5 KT LEFT CROSS WIND. SO I REMINDED MYSELF TO LEVEL THE WINGS ON LIFTOFF WHICH I DID.

RIGHT WHEN THE AIRCRAFT WANTED TO FLY AND GOT IN GROUND EFFECT, THE AIRCRAFT DRIFTED HARD AND YAWED IMMEDIATELY TO THE RIGHT, AND I ADDED LEFT AILERON INSTINCTIVELY TO CORRECT FOR THE DRIFT, AND THAT'S PROBABLY WHEN THE WING TIP SCRAPED THE GRASS.

AFTER THAT, THE PLANE WANTED TO DIVE AND I WANTED TO PROTECT THE NOSE GEAR AND PROP BY PULLING BACK ON THE STICK BUT THE STALL HORN CAME ON SO I NEUTRALIZED THE CONTROLS AND CLIMBED OUT. FLEW THE RIGHT DOWNWIND 28R, THE PLANE FLEW PERFECTLY, DID A NORMAL BASE, FINAL, AND LANDED.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR25LA139](https://openairdocs.org/ntsb/WPR25LA139), Apr 20, 2025: Diamond Aircraft Ind DA 40 (N22QT), Oakland, CA, substantial damage. Probable cause: The pilot’s failure to maintain airplane control during takeoff with a left quartering crosswind.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
