## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON APRIL 30, 2025, AT APPROXIMATELY 18:41 LOCAL TIME, THIS OFFICE WAS CONTACTED BY THE REGIONAL OPERATIONS CENTER REGARDING A HELICOPTER ACCIDENT AT WILEY POST AIRPORT (KPWA). SURVEILLANCE FOOTAGE SHOWS THE HELICOPTER APPROACHING FROM THE NORTH IN AN ATTEMPT TO LAND FACING SOUTH ON A MOBILE LANDING PAD ATTACHED TO A TRACTOR. DURING THE LANDING, THE HELICOPTER’S LEFT SKID MADE CONTACT WITH THE PAD ON THE FAR-LEFT SIDE, WHILE THE RIGHT SKID REMAINED AIRBORNE. THE HELICOPTER APPEARED TO BANK TO THE RIGHT WHICH LOWERED THE RIGHT SKID INTO THE GAP BETWEEN THE SECTIONS OF THE LANDING PAD. THE REAR OF THE RIGHT SKID CAUGHT THE LANDING PAD AND THIS CAUSED THE AIRCRAFT TO ROLL OVER TO THE RIGHT, WITH THE MAIN ROTOR BLADES STRIKING THE GROUND. THE HELICOPTER CAME TO REST APPROXIMATELY 66 FEET NORTHWEST FROM THE LANDING PAD, ORIENTED ON A HEADING OF 330 DEGREES NORTHWEST.

Probable cause: The pilot’s improper landing technique that allowed the helicopter’s right skid to get caught on the landing platform, which resulted in a dynamic rollover.
