As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

5-29-25 PIC ALONG WITH ONE PASSENGER FILED IFR FLIGHT PLAN FROM KCOU TO KPHL DEPARTURE TIME WAS 0945 AM CDT. PLANNED ALTITUDE WAS 17,000 FEET. AT ALTITUDE IN IMC CONDITIONS WITH SOME LIGHT ICE FOR APPROXIMATELY 45 MINUTES. PILOT CIRCLED UNTIL PATTERN ALTITUDE AND PERFORMED AN EMERGENCY LANDING. DURING DECENT THE ENGINE WAS NOT PRODUCING POWER. AT APPROXIMATELY 2000 FEET THE ENGINE WAS COMPLETELY SHUT DOWN DUE TO SMOKE AND OIL COMING OUT OF THE ENGINE COWL. UPON VISUAL INSPECTION OF THE ENGINE A HOLE APPROXIMATEY 2.25" X 3.5" WAS DISCOVERED JUST AFT OF THE #3 CYLINDER ON THE TOOP OF THE RIGHT-HAND SIDE OF THE ENGINE CASE. A SECTION OF APPROXIMATELY2.5" FROM THE CAMSHAFT WAS MISSING. IT APPEARS THAT THE CAMSHAFT FAILED AT THIS LOCATION. SOME OF THE BRASS MATERIAL WAS NOTED ON THE INSIDE OF THE ENGINE COWLING. ENGINE WAS SENT TO LYCOMING ENGINES FOR TEARDOWN AND EVALUATION ON 06-18-2025 AND IS SCHEDULED FOR TEARDOWN ON 7-7-25. THE PILOT FOLLOWED UP WITH A REPORT ON 8-21-25 WITH A PRELIMENARY REPORT THAT A TURBOCHARGER FAILED AT CRUISE FLIGHT. THE FAILURE OF THE TURBOCHARGER CAUSED THE ENGINE OIL TO RAPIDLY DEPLETE AND CREATE A SITUATION OF OIL STARVATION. THE OIL STARVATION RESULTED IN ONE OF THE MAIN ROD BEARINGS TO SEIZE AND THE ROD BROKE OFF AND HIT THE CAMSHAFT CAUSING PART OF THE CAMSHAFT TO BREAK OFF AND CREATE A HOLE IN THE TOP OF THE CASE. FINAL TEARDOWN REPORT IS PENDING.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
