## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON JULY 21, 2025, ABOUT 1048 ATLANTIC STANDARD TIME, A PIPER PA-28-181 AIRPLANE CRASHED SHORTLY AFTER DEPARTING RUNWAY 09 AT FERNANDO LUIS RIBAS DOMINNICCI AIRPORT (TJIG), SAN JUAN, PUERTO RICO. THE AIRPLANE WAS REGISTERED TO INTER AMERCAN UNIVERSITY OF PUERTO RICO SCHOOL OF AERONAUTICS, AND THE FLIGHT WAS CONDUCTED UNDER THE PROVISIONS OF TITLE 14 CODE OF FEDERAL REGULATIONS PART 91.

THE STUDENT PILOT STARTED THE TAKEOFF ROLL FOR A SOFT-FIELD TAKEOFF AND CLIMB. ONCE THE AIRCRAFT BECAME AIRBORNE AT AN EARLY SPEED, THE AIRCRAFT YAWED LEFT. AS PER THE SURVEILLANCE VIDEO OBTAINED, THE AIRCRAFT WAS OBSERVED CLIMBING WITH EXCESSIVE NOSE UP ATTITUDE AND TURNING TO THE LEFT. THE AIRCRAFT CONTINUED ITS LEFT TURN UNTIL IT STALLED, LATER IMPACTING A PARKED TRAILER WITH THE AIRCRAFT LEFT WING WHICH DETACHED FROM THE FUSELAGE UPON IMPACT WITH THE GROUND, AND CONTINUED ITS TRAJECTORY UNTIL IT CAME TO REST ON THE GROUND AT A NEAR TRUCK PARKING AREA APPROXIMATELY 750 FEET NORTH OF RUNWAY 09. THE AIRCRAFT WAS DESTROYED SUBSEQUENTLY, AND THE STUDENT PILOT REPORTED NO INJURIES AND WALKED OUT OF THE ACCIDENT AIRCRAFT BY HERSELF UNAIDED.

NOTE FOR ATQA AUTOMATION: WHEN BLOCK 30 IS CHECKED WITH "YES" OR "NO", THE AUTOMATION DOES NOT ALLOW THE USER TO UNCHECK THIS RADIO BUTTON. IIC SELECTED "NO" IN BLOCK 30 BY ERROR. THIS WAS NOT AN AIR CARRIER FLIGHT.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB ERA25LA275](https://openairdocs.org/ntsb/ERA25LA275), Jul 21, 2025: Piper PA28 (N4187U), San Juan, PR, substantial damage. Probable cause: The student pilot’s excessing pitch attitude during takeoff, which resulted in a loss of aircraft control and aerodynamic stall.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
