^PRIVACY DATA OMITTED^WAS OPERATING UNDER PART 135 AS AN AIR MEDICAL TRANSPORT FLIGHT. THE ACCIDENT OCCURRED DURING THE AIRCRAFT’S SECOND MISSION AND THIRD FLIGHT OF THE DAY. THE ACCIDENT OCCURRED DURING THE APPROACH TO RUNWAY 36. EVIDENCE SUGGESTS A POSSIBLE GO-AROUND WAS INITIATED, AS THE LANDING GEAR WAS RETRACTED AND FLAPS WERE FOUND IN THE FULL-UP POSITION. WEATHER AT THE TIME INCLUDED PARTLY CLOUDY SKIES WITH LEFT CROSSWINDS AT 15 KNOTS, GUSTING TO 30 KNOTS.

THE PILOT IN COMMAND (PIC) HELD A CURRENT FIRST-CLASS MEDICAL CERTIFICATE AND REPORTED 19,671 TOTAL FLIGHT HOURS AT THE TIME OF HIS LAST MEDICAL. THE SECOND IN COMMAND (SIC) ALSO HELD A CURRENT FIRST-CLASS MEDICAL AND REPORTED 186 TOTAL FLIGHT HOURS. NO ENFORCEMENT INFORMATION SYSTEM (EIS) HISTORY WAS FOUND FOR EITHER PILOT.

THE AIRCRAFT CAME TO REST APPROXIMATELY 990 FEET WEST OF RUNWAY 36, ON THE EDGE OF THE AIRPORT PERIMETER ROAD. A POST-CRASH FIRE CONSUMED THE FORWARD TWO-THIRDS OF THE AIRCRAFT.

FAA INSPECTORS ARRIVED ON SCENE AT APPROXIMATELY 2200 LOCAL TIME ON AUGUST 5, 2025. A PHYSICAL INSPECTION OF THE WRECKAGE REVEALED NO INDICATIONS OF PRE-IMPACT MECHANICAL ANOMALIES.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR25FA240](https://openairdocs.org/ntsb/WPR25FA240), Aug 5, 2025: Beech 300 (N534AW), Chinle, AZ, destroyed, 4 fatal, investigation ongoing

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
