## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

ON SEPTEMBER 15, 2025 AT 2315 ZULU, ^PRIVACY DATA OMITTED^, AN AIRBUS A388, WAS TAXING FOR DEPARTURE ON TAXIWAY A, AT DANIEL K. INOUYE INTERNATIONAL AIRPORT, HONOLULU, HAWAII (HNL).

DURING THE TAXI, ANA181 STRUCK A STRING OF CARGO CONTAINERS THAT HAD ROLLED INTO THE MOVEMENT AREA. THE PILOT REPORTED SHORTLY AFTERWARDS THAT THEY WERE HAVING AN ISSUE WITH THE #4 ENGINE. MAINTENANCE STAFF WAS CALLED AND CONFIRMED DAMAGE TO THE ENGINE COWLING AND TOWED THE AIRCRAFT BACK TO THE GATE.

LATER REPORTS FROM THE CARGO OPERATOR STATE THAT THE CARGO CONTAINER HAD BEEN PARKED IN A STAGING AREA AND THE BRAKE HAD BEEN SET BUT SHORTLY THEREAFTER, THE BRAKE MECHANISM HAD FAILED, ALLOWING THE CONTAINER TO ROLL ON ITS OWN INTO THE MOVEMENT AREA.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
