## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

DURING A PART 91 INSTRUCTIONAL PROFICIENCY FLIGHT AT KICT, THE AIRCRAFT COMPLETED A PLANNED NO-FLAP LANDING AND SUBSEQUENT TOUCH-AND-GO. DURING CLIMB OUT ON THE UPWIND LEG AT APPROXIMATELY 700–800' AGL, THE AFT CARGO DOOR UNLATCHED AND BLEW OPEN IN FLIGHT. THE DOOR STRUCTURALLY IMPACTED THE AIRFRAME, CAUSING SUBSTANTIAL COSMETIC/STRUCTURAL CRACKING TO THE RH AFT WINDOW AND THE SURROUNDING FUSELAGE BULKHEAD ATTACH POINTS. THE OPEN DOOR SAFETY SWITCH SUCCESSFULLY TRIGGERED AN INOPERATIVE FLAP CONFIGURATION. POST-FLIGHT INSPECTION REVEALED BOTH COCKPIT DOOR HANDLES/LATCHES WERE STILL PHYSICALLY SELECTED TO THE ENGAGED/CLOSED POSITION.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB CEN26LA163](https://openairdocs.org/ntsb/CEN26LA163), Apr 3, 2026: Textron T206H (N411CV), Wichita, KS, substantial damage. Probable cause: The flight crew’s failure to ensure that the rear cargo door was properly latched before flight.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
