## Narrative

As the FAA filed it, in capitals; the FAA replaces private details with ^PRIVACY DATA OMITTED^.

PILOT STATES SHIMMY CHECK WAS POSITIVE AND SEAT WAS LOCKED BEFORE TAKEOFF ROLL. SEAT THEN BECAME DISENGED AND SLID BACK DURING ROTATION CAUSING A LOSS OF CONTROL DUE TO NO LONGER BEING ABLE TO REACH THE CONTROLS. DURING THE ONSITE EVALUATION BY THE INSPECTOR, AFTER THE ACCIDENT, IT WAS NOTED THAT WHILE REPOSITIONING THE SEAT TO TAKE PICTURES OF THE SEAT TRACKS, THE SEAT WOULD LOCK WHILE ROLLING INTO PRESET LOCKING POSTIONS BUT WOULD CONTINUE TO MOVE WHEN OCCUPANT WEIGHT WAS SHIFTED OR WHEN SEAT WAS SHIMMIED. THE SEAT IN QUESTION WAS ALSO EVALUATED BY A CIRRUS REPRESENTATIVE WHO DID NOT SEE ANY DIRECT PHYSICAL EVIDENCE OF SEAT FAILURE AND THEY WERE ABLE TO CONFIRM THE SEAT LOCKS IN PLACE. NON-MANDATORY CIRRUS SB2X-25-31R1 WAS ISSUED 12 AUGUST 2024 DUE TO AWARENESS OF A CONDITION WHERE CREW SEATS MAY SLIDE IF ALL SEAT PINS ARE NOT FULLY ENGAGED. SB2X-25-31R1 WAS NOT PERFORMED ON ^PRIVACY DATA OMITTED^.

## The NTSB's investigation of this event (1)

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.

- [NTSB WPR26LA159](https://openairdocs.org/ntsb/WPR26LA159), Apr 13, 2026: Cirrus Design SR22T (N285AH), Fullerton, CA, substantial damage. Probable cause: The pilot’s loss of control during takeoff, due to the seat sliding aft.

From the FAA Accident/Incident Data System. Reports are matched to registry aircraft by N-number and event date, checked against the reported make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model. For accidents, the NTSB's own investigation is the fuller record.
