## Probable cause

THE PILOT-IN-COMMAND'S FAILURE TO MAINTAIN AIRPLANE CONTROL DURING THE TAKEOFF ROLL, AND FAILURE OF NOT ATTAINING AIRSPEED ON TAKEOFF.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Overrun, takeoff - roll/run
  - aircraft control - not maintained, pilot in command (cause)
  - airspeed - not attained, pilot in command (cause)
- On ground/water encounter with terrain/water, takeoff - roll/run

## Analysis

DURING THE TAKEOFF ROLL, THE PILOT LOST CONTROL OF THE AIRPLANE. THE AIRPLANE EXITED THE DEPARTURE END OF THE RUNWAY INTO THE TUNDRA.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19900922041609A](https://openairdocs.org/incident/19900922041609A), Sep 22, 1990: Dhav DHC3, King Salmon, AK, Substantial damage. Wind shift on takeoff. Unable to lift until end of runway. Gear hit tundra vegetation. Failed to timely abort.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
