## Probable cause

WAS THE PILOT-IN-COMMAND'S FAILURE TO MAINTAIN DIRECTIONAL CONTROL OF THE AIRPLANE DURING THE TAKEOFF RUN. CONTRIBUTING TO THE ACCIDENT WAS THE ROUGH WATER.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, takeoff - roll/run
  - terrain condition - water,rough (contributing factor)
  - directional control - not maintained, pilot in command (cause)
- Dragged wing,rotor,pod,float or tail/skid, takeoff - roll/run
- Nose over, takeoff - roll/run

## Analysis

DURING THE TAKEOFF ON THE WATER, THE PILOT LOST CONTROL OF THE AIRPLANE AND THE LEFT WING TIP STRUCK THE WATER. THE AIRPLANE NOSED OVER ON ITS BACK AND SANK INTO THE LAKE.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19910814034309A](https://openairdocs.org/incident/19910814034309A), Aug 14, 1991: Dhav DHC2MK1, Ugashik, AK, Substantial damage. On crosswind water takeoff upwind wing came up. Left wing hit water. Cartwheel. Nosed over.

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
