## Probable cause

THE PILOT'S FAILURE TO PERFORM A GO AROUND. CONTRIBUTING TO THE ACCIDENT WAS THE ICY RUNWAY CONDITION AND THE FAILURE OF THE FSS PERSONNEL TO PROPERLY ISSUE AND DISSEMINATE A NOTAM CONCERNING THE ICY RUNWAY CONDITION.

## Sequence of events

In NTSB's coding before 2008 (https://www.ntsb.gov/GILS/Documents/codman.pdf).

- Loss of control - on ground/water, landing - roll
  - airport facilities,runway/landing area condition - icy (contributing factor)
  - go-around - not performed, pilot in command (cause)
  - notams - not issued, atc personnel(fss) (contributing factor)
- Overrun, landing - roll
- On ground/water collision with object, landing - roll
  - object - tree(s)

## Analysis

THE AIRPLANE LANDED ON AN ICE AND WATER COVERED RUNWAY, THEN WAS UNABLE TO STOP BEFORE RUNNING OFF THE DEPARTURE END. ON THE PREVIOUS DAY, A PILOT HAD REPORTED, TO A NEARBY FAA FSS, THAT THE RUNWAY WAS COVERED WITH GLARE ICE AND WATER AND THAT THE BRAKING ACTION WAS NIL. THIS INFORMATION WAS NOT PUT IN A NOTAM, AND THE ACCIDENT PILOT WAS NOT AWARE OF THE RUNWAY CONDITION PRIOR TO LANDING.

## FAA reports on this event (1)

The FAA's own accident and incident reports, matched to the aircraft by N-number and date (OpenAirDocs).

- [Accident 19911225057199A](https://openairdocs.org/incident/19911225057199A), Dec 25, 1991: Piper PA31350, Tatitlek, AK, Substantial damage. Unable to stop on ice covered runway. Ran off end. Collided with stumps. Pilot advised fss of icy runway. No NOTAM

From the NTSB's public API. Aircraft are matched to registry aircraft by N-number and event date, checked against the reported serial number, make and model; the registry's history has gaps, so older matches can be wrong. Compare the reported make and model.
